Domiciliary physiotherapy
Domiciliary physiotherapy is a physiotherapy treatment given at a home of a patient. We target this services to disable people, with deficits in a self-care. Services can be given maximum for 80 days in the year.
Referral can be obtained by people who suffer from:
- focal brain injury (embolism, stroke)
- hard injury of a central and circular nervous system (5th grade in Rankin scale)
- injury of a spinal cord
- chronic ilnesses (Parkinson, polymyositis, tubers, arthritis)
- degeneration of hip joint or knee joint
- lower extremities injuries.
Domiciliary physiotherapy treatment is offered for the National Health Fund policy holders who are granted referral form the doctor of following specialisation:
- reumatology,
- neurology,
- orthopaedics,
- medical rehabilitaion,
- balneology and physical medicine.
or by a family doctor (referral to the reabilitaion clinic).
Referral for domiciliary physiotherapy treatment needs to provide:
- a stamp of an institution that is issuing the referral (number of a contract wih the NFZ must be included)
- surname, name, PESEL and address of a patient
- diagnosis
- code of a illness in accordence with the ICD- 10 classification
- subscription of disfunctions
- current and passed illnesses (types of medicines, inforamtion on hart pacemaker)
- type of a treatments that are to be applied and areas of the body (quantity and parameters of treatments)
- doctor stamp, signature and date of issue.
